Add Checkboxes to Forms
I need checkboxes to form. We need it in order to make sure client checked the terms and agreement before form submission.

HI, I have already contacted support and here's what he told me
"As this was built in the Form editor, there isn't an option to add a checkbox. If you built this form out on a Page, then you would have more options to customize the form, like potentially adding a checkbox". I need this form to be embedded and not on Ontra Page.
