Can anyone tell me if there is a smartform that allows me to create a fill-in-the blank series of questions?
I'm needing to create a landing page that allows viewers to complete a series of questions with their own answers (not drop down) and submit to be considered for a free coaching session.
How do I do this? Hoping I don't have to interface with something like SurveyMonkey.

Custom fields types don't include "fill in the blank specifically". We'd recommend that you create a text field to let the prospect give you their answer. Use the field label to create the "fill in the blank" feel.
Example:
Custom Field Name: Interests
Field Label: My interests are __________ (fill in the blank).
