Check out our Systems Setup Wizard for a simple survey, "Smiley Survey and Testimonial Generator". It will install a custom object (a separate database) linked to your contacts that stores their survey responses. You can use that example as a template for a more robust system.
Navigate to Systems and click the "+ New System" button to reveal the System Setup Wizards!
