Upsell forms - Pricing options

This is a request to allow multiple payment plan options for upsell forms. "If OP are going to be a serious player in the membership space, this should be a basic function. A lot of people promote a self-liquidating offer that then has an upsell to a membership site. In any membership site promotion you're going to offer the monthly payment amount or a discounted upfront amount for 1 year worth of membership."
